Special Advance Directive
The Advance Directive for Alzheimer's Disease is a specific advance directive to be attached to your general advance directive. (The general form gives only superficial direction for dementia).
Different people may want different 'trigger points' re the operation of the directive, according to the stage of the disease and their personal philosophy. You can select your trigger point in the fifth paragraph. Seek advice from your doctor if this is not clear to you.
You should clearly understand that this directive is a model; it contains options that you can select; you can delete any part that you wish, or add any words that you wish, but you should seek advice that such changes do not destroy the clarity of the document.
The last paragraph indicates your mental state when you are completing the directive. One of these three categories will need to be chosen when you complete the document.
Appoint an Agent As Well
Since it is very likely that you will be mentally incompetent when this directive may need to be acted upon, it is essential that you have also appointed an agent (Medical Enduring Power of Attorney) to act for you. This person is selected by you to make decisions on your behalf if you become incompetent, and they are legally protected in making those decisions, particularly if they act according to your instructions as set out in the advance directive.
