Whether you are infirm or in excellent health, you can use an Advance Directive to make your medical treatment preferences known should a sudden, unexpected catastrophic illness befall you which may render you unable to make your wishes known.

In addition, if you already have — or have reason to fear — a specific life threating illness, you should also attach another Advance Directive form that is specific to that illness. The general Advance Directive covers a wide range of illnesses but it is valuable to specify your wishes in greater detail for certain illnesses to ensure your wishes are known.
