If you are applying for Voluntary Assisted Dying, at the final stage of the application process, you complete a form formally requesting VAD. This has to be done in the presence of your coordinating practitioner and two other people who act as Witnesses.
Ideally, patients find their own witnesses, but this is not always possible. DWDV is able to provide you with assistance, should you require it. We have a pool of trained Witnesses who may be able to assist.
