If you are applying for Voluntary Assisted Dying, you need to appoint a Contact Person who will return unused medication to the pharmacy if you die before taking the medication or decide not to take it.
One stage of the application process also requires that you complete a form formally requesting voluntary assisted dying. This has to be done in the presence of your coordinating practitioner and two other people who act as Witnesses.
